'Death chambers': Why coaching centre tragedies keep repeating

On June 22, smoke began pouring out of a three-storey commercial building in Lucknow's Aliganj. Within minutes, people trapped inside were banging on windows, smashing glass and desperately searching for a way out.
Some tried climbing down using ropes and cables while others reportedly jumped from upper floors.
