Nagpur police face scrutiny after NEET UG aspirant Akanksha Chaturvedi's suicide note was not added to the case papers, IO gets show cause notice.

A police investigation into the death of an 18-year-old NEET-UG aspirant has come under scrutiny after it emerged that her purported suicide note was not attached to the case papers, prompting disciplinary action against the investigating officer.
A senior police official said on Saturday that a show cause notice has been issued to Investigation Officer Nikhil Tabhane of Ambazari police station for allegedly failing to include the handwritten note, believed to have been written by the deceased student, in the official case file.
“The suicide note was mentioned in the station diary entry. However, it was not attached to the case papers," said DCP (Zone 2) Nityanand Jha.
Another official said the note was not referred to in any of the documents related to the case, and even DCP Jha was initially unaware of its existence. Once the omission came to light, senior officers conducted an internal inquiry to determine responsibility and submitted a report to the DCP, following which the show cause notice was issued.
The case concerns NEET-UG aspirant Akanksha Chaturvedi, 18, who allegedly hanged herself in her room in Nagpur on May 20, days after appearing for the exam on May 3. She hailed from Madhya Pradesh and had been preparing to become a doctor.
In the emotional note, Chaturvedi purportedly expressed her fear and anxiety over having to sit for a retest ordered by the National Testing Agency after the original exam was cancelled amid allegations of a paper leak. She reportedly wrote that there was “no guarantee" she would score good marks again.
The incident gained nationwide attention after Congress leader Rahul Gandhi highlighted the case on the social media platform X.
The suicide note was discovered only days after her death, when her family members were going through her books and study material. They found the handwritten note and submitted it to the Ambazari police on June 1.
“Mummy, Papa… you had trust in me that I would study and become a doctor. But I do not have the courage to appear for a retest. In the first test, I was about to score good marks. But there is no guarantee that I will score good marks again. Sorry, Mummy, Papa, I have ruined everything for both of you," the note reads.
Her father, Krishna Kumar Chaturvedi, a small farmer who also worked as a cook in Nagpur to support the family, told police that his daughter had returned from the examination feeling optimistic and expected to score more than 650 marks.
However, the family later noticed a significant change in Akanksha’s behaviour. She appeared increasingly disturbed after reports emerged about a paper leak and the rescheduling of the exam.
An official from Ambazari police station said statements of family members have been recorded and that a further probe is underway. All aspects mentioned in the suicide note are being examined, the official added.
